All participants, and parents/legal guardians of minors, are required to read this document. It contains important information about your course including, but not limited to:

- What you will need and what is provided
- Directions to, and information about, your training at the pool and lake
RSTC Medical Statement and Divers Medical Questionnaire
All participants are required to print, read, and complete this form.
Participants under the age of 18 must also have a parent/legal guardian sign this form.
Note: If you are able to answer "YES" to one or more of the questions on the Divers Medical Questionnaire, then you must obtain a physician's sign-off on this form prior to your training in the pool.
Liability Release and Assumption of Risk Agreement
All participants are required to print, read, and complete this form.
Participants under the age of 18 must also have a parent/legal guardian sign this form.
Emergency Treatment Consent Form
Only for participants under age 18.
Parents/Legal Guardians of participants under the age of 18 are required to print, read, and complete this form.
Youth Diving: Responsibilities and Risks Flipchart
Only for participants under age 12.
Parents/Legal Guardians are required to review this flipchart with participants aged 10 or 11.
Youth Diving: Responsibilities and Risks Acknowledgement
Only for participants under age 12.
Parents/Legal Guardians of participants aged 10 or 11 are required to read and sign this form with your child after reviewing the flipchart.
