Please print, read, fill out, and sign the following forms as required and bring them to your course sessions as instructed below.

Student Diver Guide Book  [Download]
All participants, and parents/legal guardians of minors under age 18, are required to read this document. Keep it handy throughout your training. It contains important information about your course including, but not limited to:
- Terms and Conditions
- Documentation and Medical requirements
- How to proceed through the course
- What you will need and what is provided
- Directions to, and information about, your training at the pool and lake

Student Diver Forms Packet  [Download]
All participants are required to print, read and sign these forms. The packet contains two (2) sets of all the forms you will need. One set is to be provided to your instructor on orientation night, the other to your instructor at your open water dives. Participants under the age of 18 must also have a parent/legal guardian sign these forms. The packet contains two of each of the following forms:
- Open Water Diver Course Record and Referral Form
- Medical Statement and Divers Medical Questionnaire
- Standard Safe Diving Practices Statement of Understanding
- Liability Release and Assumption of Risk Agreement

Emergency Treatment Consent Form  [Download]
Required for participants under age 18.
Parents/Legal Guardians of participants under the age of 18 are required to print, read, and complete this form. Please make two copies, one for the instructor on orientation night, the other for the instructor at the open water dives.

Youth Diving: Responsibilities and Risks Flipchart  [Download]
Required for participants under age 12.
Parents/Legal Guardians are required to review this flipchart with participants aged 10 or 11.

Youth Diving: Responsibilities and Risks Acknowledgement  [Download]
Required for participants under age 12.
Parents/Legal Guardians of participants aged 10 or 11 are required to read and sign this form with your child after reviewing the flipchart. Please make two copies, one for the instructor on orientation night, the other for the instructor at the open water dives.
